Position Summary:
The Transition Coach will take ownership of providing individualized, thoughtful coaching for the young people who transition out of HeartShare St. Vincent’s Services’ Healing Home Residences. The Transition Coach will provide comprehensive support for youth during and after the transition to independent living. Comprehensive support will primarily revolve around continued education and employment coaching, but will also include independent living skills, socioemotional support, housing support, assistance with building/maintaining the youth’s support network, and more. This coach should have innovative ideas for how to support youth who are not as motivated or may struggle academically, have experienced trauma, have mental health disorders, are cautious in building trust, and may seek alternative roads to independent success. This Coach should also firmly believe that there is a vocational or educational path for each youth. This Coach will utilize HSVS’ current education and employment coaching systems and models while working to include adjustments to support the specific needs of our youth living who transition out of the Healing Homes. This Coach will partner with each youth’s Success Coach, Housing Specialist, Healing Home Staff, and any other individuals who are supporting each youth. This Coach will be responsible for collecting and analyzing data to drive the work with the youth they support. This Coach will directly report to the Success Coach Supervisor and is a part of the Office of Youth Development.
Qualifications:
Minimum Education:
Bachelor’s degree from a four-year college or equivalent
Preferred Education:
Bachelor’s degree from a four-year college or equivalent
Experience Required:
· Experience working in the education and other employment systems.
· Experience with supporting Individualized Education Plans and 504s.
· Experience working with youth with serious emotional disturbance
· Experience working with vocational and alternative high school programs
Experience Preferred:
· Experience working with OMH (Office of Mental Health) population, especially with children, teens, and young adults.
· Experience working with families of youth with serious emotional disturbance
· Experience working with post-secondary programs.
· Experience leading workshops and facilitating groups (adults and youth).
Skills Required:
· Excellent verbal and written communication skills are a must.
· Computer Skills — excellent knowledge of Microsoft Office and databases including Microsoft Access.
· Operating Skills — flexibility in working on assignments, staying on task and meets deadlines, strong analytical skills, creative and critical thinker, and ability to stay organized. Experience and confidence with recruiting, training and managing teams, working with data, and ability to prioritize multiple responsibilities.
· Personal & Interpersonal Skills — self-motivated, team player, able to relate with teens and young adults, able to manage stress, and able to see value in and promote diversity in others. Passionate about delivering friendly, flexible, compassionate, and effective services to youth in foster care. Ability to work collaboratively with internal and external partners.
Primary Responsibilities and Essential Functions of Position:
1. Build relationships with the youth who are transitioning out of Healing Homes.
2. Develop collaborative relationships with the team in our Healing Homes, Permanency Units, Education, and Youth Development programs.
3. Support a roster of youth as determined by the Director of Success and Housing Services and the Healing Home Leadership.
4. Conduct educational assessments, identify educational strengths and challenges, and draft plans for youth and their teams.
5. Use evidence-based assessments paired with internal structures to drive support of youth.
6. Review and revise current education resources to provide more individualized support for youth.
7. Actively prepare, participate, and advocate in youth’s education team meetings and appointments.
8. Develop relationships with potential employers and school opportunities.
9. Connect youth with community partners and champions while in care and as they prepare to transition out of care.
10. Organize, facilitate, and/or identify academic and vocational groups and exploration opportunities (i.e. job fairs, college tours, shadowing opportunities).
11. Participate in the service planning and review process.
12. Provide crisis intervention and behavior management.
13. Provide reports for the youth and their teams as determined by the Director of Success and Housing Services and Healing Homes Leadership.
14. Maintain accurate, up-to-date educational and employment data for the youth served.
15. Partner with youth to explore job and internship opportunities as well as help them prepare for job interviews
16. Employ HSVS’ trauma-sensitive practices within the coaching model.
17. Support projects and initiatives for the Youth Development and Education Teams.
18. Meet targets as determined by the Director of Success and Housing Services and the Healing Home Leadership Team.
19. Assist youth in building a healthy network of individuals for post-transition supports
